• A valid passport (should be valid at least six months after the expiration of the issued visa) and any old passports (if applicable)
   
• Triplicate copies of the peso deposit slip for the non-refundable application fee
   
• Letter/Certification from the Philippine company
   
• Confirmed roundtrip booking certificate (only print outs from the airlines or a travel agency is accepted and should be at least 15 working days prior to the departure date)

• A valid health and travel insurance
• Invitation from the Swedish company (could be sent directly to the Embassy)

C. TRAINING PROGRAMMES, COURSES OR SEMINARS (Organized by SIDA)
(If you are accepted to participate in training, seminars, etc. organized by SIDA in Sweden, please apply for this visa.)

Original:

• Application for Schengen Visa Form 119031
   

• Filled-in "Appendix A" Form 210021
   

• Two (2) recent, colored passport-size pictures
   

• A valid passport (Should be valid at least six months after the expiration of the issued visa) and any old passports (if applicable)
   

• Confirmed roundtrip booking certificate (only print outs from the airlines or a travel agency is accepted and should be at least 15 working days prior to the departure date)

Original plus one copy:

• Original acceptance letter from the Swedish course administrator
   

• If the visa is approved, the applicant is required to present the passport, a confirmed roundtrip ticket and a valid health and travel insurance before the visa is issued.

D. GENERAL TOURISM
(If you are going to Sweden on a tour, cruise, etc., please apply for this visa.)

Original:

• Application for Schengen Visa Form 119031
   

• Filled-in "Appendix B" Form 212031
   

• Filled-in "Appendix D" Form 201031
   

• Two (2) recent, colored passport-size pictures
   

• A valid passport (Should be valid at least six months after the expiration of the issued visa) and any old passports (if applicable)
   

• Triplicate copies of the peso deposit slip for the non-refundable application fee
   

• Confirmed roundtrip booking certificate (only print outs from the airlines or a travel agency is accepted and should be at least 15 working days prior to the departure date)

Original plus one copy:

• Proof of relationship: marriage and/or birth and/or baptismal certificates (if travelling with an immediate family member)
   

• Confirmation from the local travel agency's counterpart in Sweden to be sent directly to the Embassy via fax or e-mail (your booking for a particular package tour, sightseeing or similar)
   

• Confirmed hotel booking from Sweden to be sent directly to the Embassy via fax or e-mail
   

• Day-to-Day Tour itinerary
   

• Proof of sufficient funds to travel
   

• If employed, employment certificate (stating date of employment, salary, permanent or contractual) and certificate of vacation leave (with or without pay, when are you being expect to report back to work), if self employed, business permits/documents
   

• If the visa is approved, the applicant is required to present the passport, a confirmed *roundtrip ticket and a valid health and travel insurance before the visa is issued. (*Restricted/non-rebookable/non-refundable air tickets must not be purchased before visa is approved)

• Please note that the Embassy might request for additional documentation and information if deemed necessary.

E. FAMILY VISIT - BY PARENTS, CHILDREN OR SIBLINGS
(If you are going to Sweden to visit parents, siblings or spouses, please apply for this visa.)

Original:

• Application for Schengen Visa Form 119031
   

• Filled-in "Appendix B" Form 212031
   

• Filled-in "Appendix D" Form 201031
   

• Two (2) recent, colored passport-size pictures
   

• A valid passport (Should be valid at least six months after the expiration of the issued visa) and any old passports (if applicable)
   

• Confirmed roundtrip booking certificate (only print outs from the airlines or a travel agency is accepted and should be at least 15 working days prior to the departure date)

Original plus one copy:

• Filled in "Appendix E" Form 213031 (Referentbilaga) clearly indicating the month and year the applicant will be entering Sweden and the duration of the stay
   

• Invitation or Affidavit of Support, whichever of the two, legalised by Notary Public in Sweden
   

• Person Bevis, if not applicable, proof of the relative's residence permit in Sweden or Swedish citizenship
   

• Proof of sufficient funds to travel (if applicant will shoulder the expenses)
   

• Proof of relationship: marriage and/or birth and/or baptismal certificates

• If employed, employment certificate (stating date of employment, salary, permanent or contractual) and certificate of vacation leave (with or without pay, when are you being expect to report back to work), if self employed, business permits/documents
