A premium is the upfront cost of an insurance policy, meaning the amount you are required to pay to receive coverage from an insurance underwriter. By providing the premium to an insurance underwriter, you are entering into a contract in which they (the insurance provider) assumes a majority of the risk should you require medical attention while traveling. The premium is determined by a number of factors but there are two elements that influence the premium most: age and risk. Since most underwriters do not require a pre-screening physical for the acquisition of a policy, they rely on the age of the policyholder to help determine an appropriate policy premium. Apart from age, risk also plays a major factor in determining a policy premium. The more risk you are willing to assume, the lower your premium will be. Visa Versa, the less risk you are willing to assume (in turn, requiring the underwriter to assume a higher risk) the higher your policy premium will be.
